Please provide an order form per brick

All bricks will be a finished brick pavers which is a brick that has a smooth surface and beveled edges with spacers on the sides and is basically a "pretty brick". This brick is available in 4x8 with a thickness of 2 1/4".

For this size brick, up to 21 characters per line and 4 lines is allowed but 18 characters and 3 lines are recommended. A character means any letter, number, space or punctuation mark. Each brick ordered is $100.00.

A separate order form is required for each brick order.
